You're missing the "trans" aspect. Transgender means it's transitory- if a transgender person still has their original genitals, it usually means they are in transition before having a sex change, which is also expensive, and so sometimes it doesn't happen overnight.

There is a different between gender and sex. Gender is how the person considers himself based on social structure. Sex is how he is identified biologically. So a trans person who still has parts of the opposite sex identifies himself based on his gender.
